Aleksandra walked up to the roster board to check her duties and saw that it was pairing her with Dr Cullecher again. She received a page to meet with him on third floor and proceeded to walk the stairs with Davina. Aleksandra was greeted by Dr Cullécher and told her to follow him as he led her to his office and got her to grab his autoinjector pens while he grabbed his eyePocket before they went to the first patient which was simply having mild allergy symptoms. Dr Cullécher simply got her to administer an anti-allergen after checking their vital before moving to the second patient which was anaphylactic which Aleksandra used an autoinjector pen. These objective repeat with the third patient for being another anti-allergen and the fourth being another anaphylactic patient.
They got to the fifth patient which turns out to the African-American mobster, Navy Marcus, as he was being harassed by the Bayou Boys as he was suffering a peanut allergy attack. Dr Cullécher tries to order the rednecks out of the patient's room, but one of them draw out a knife and proceeded to slit Dr Cullécher forcing Aleksandra to act and put pressure on his wound before several surgeons came to take him to OR immediately where Aleksandra became enraged and proceeded to began use an autoinjector on Navy before beating down on Billy King. She managed to disarm him and proceeded to kill him by stabbing him with his own knife before going after Bryan Duke and Byron Earl. After stabbing them to death; Bobby Count tried to draw his weapon her, but she stabbed him in the arm and took his pistol from him before delivering an execution-style shooting on Bobby in which she shot him in the shoulders before shooting him in the neck.
Navy began thanking her when he heard a security coming and grabbed the gun as a the guard pointed his weapon at her and shot him in the head killing him before telling Aleksandra to sit down and relax. Later the police arrive as Navy was led away in handcuffs with Tracey Park checking on her. The President of the New Bourbon East Hospital, Demetrius Penseur, met up with Aleksandra and told her that he was giving her paid time-off, no discussion. Aleksandra wordlessly left the hospital.
